1. Organize all documents. To apply online for an Indiana birth certificate, you will need to know the birthplace, date of birth, the certificate holder's name, and the parents' legal names. In some cases, you will also need to supply the mother's maiden name as well as the name of the hospital where the birth took place.


2. Organize additional documentation for future use. If you are not listed on the birth certificate and do not have a close relative listed either, you need to supply VitalChek with more information of your entitlement to the certificate. 3. Visit the VitalChek website and enter the required information. When applying for your own birth certificate, the website will prompt you for your name, the date and place of birth as well as your reason for obtaining the birth certificate. If you are an immediate family member, you can purchase an Indiana birth certificate if you are named on the certificate or your close relative is; for example, a grandmother can obtain a copy of her grandchild's birth certificate only if her child is named on the document as the mother or father.
