Get A Birth Certificate In Indiana

Because birth certificates are important legal documents that serve as the foundation for many other legal documents such as drivers licenses and passports, Indiana imposes certain restrictions on the availability of a copy of someone else's birth certificate. You can always obtain a copy of your own birth certificate, however, as long as you can produce acceptable identification. The process for obtaining an Indiana birth certificate is relatively simple and straightforward.


Instructions


1. Determine if you are eligible to apply for an Indiana birth certificate. You must be the person listed on the birth certificate or the spouse, parent, legal guardian, sibling, aunt, uncle, child or grandchild of the person whose birth certificate you are requesting. Step-parents may apply only with permission form the legal parent, and federal and social agencies may apply as long as appropriate grounds are given. Otherwise, permission from the Indiana State Registrar will be required.


2. Photocopy all required identification. Indiana maintains a list of acceptable forms of identification (see References section). Some forms of identification may be used alone, and others may be used only in conjunction with other forms of identification. If you are requesting a birth certificate for yourself, or if you are listed as a parent on the birth certificate, you will only have to provide acceptable identification. Other parties will require additional documentation (a copy of a marriage certificate for a spouse, for example).


3. Download Indiana State Form 49607, Application for Search and Certified Copy of Birth Record, and fill it out. You will need to know the full birth name of the person whose birth certificate you are requesting, as well as the city, county and hospital of birth. You will also need to know the date of birth and the parents' names (including the mother's maiden name). You will have to provide your contact details and reveal how the birth certificate will be used. Do not forget to sign the application.


4. Make out a check or money order for $10 (plus $4 for each additional copy) to the Indiana State Department of Health.


5. Mail Form 49607, the check or money order and a photocopy of your identification to Vital Records, Indiana State Department of Health, P.O. Box 7125, Indianapolis, IN 46206-7125. You will receive a certified copy of the birth certificate in three to four weeks.
