Add a Parent to a Birth Certificate


There are many reasons why a father's name is not added to a birth certificate at the time of the birth. The original procedure of adding the father's name to the birth certificate generally takes place at the hospital. It is called a declaration of paternity. It is always beneficial to have both parents named on the birth certificate. Procedures of adding a parent after the initial birth vary from state to state.


Instructions


1. Request a Recognition of Parentage form. The name may vary from state to state, but it is the same form given at the hospital and it is available from your local child support agency.


2. Complete the form and have it notarized. You should not make any corrections on the form, so you may want to pick up two to begin with in order to have a spare.


3. Take the completed form with your proper state identification to your local Registrar or Department of Health, depending on your state. You can call the hospital where the child was born to find out the correct place to file the changes.


4. Pay the fee request the changes, and an original birth record with changes will be mailed to you within two to six weeks.
